- Find out if your insurance can cover your treatment with a GLP1 medication (ex. Ozempic, Trulicity, Mounjaro), or an SGLT2 (ex. Jardiance, Invokana, Steglatro).

Immigration Exams
Dr. Gougian is a US Civil Surgeon approved to perform immigration examinations for individuals seeking permanent residency (Green Card) as required by the USCIS. These appointments are available most Mondays and Fridays from 1PM - 4PM. Online scheduling is preferred.
The fee for the examination is $495.
The fee includes a physical exam, collection and processing of labs, a review of vaccine records, and completion of I-693 form.
Fee is payable by credit card. We do not accept checks or cash.
We do not accept insurance for the exam.
There is a $50 discount if using insurance to pay separately for your labs.
There is a $100 discount for refugee or asylum seekers (applicants who do not need labs).
