Featured Service: Immigration Exam

Dr. Gougian is a USCIS Civil Surgeon approved to perform immigration exams for individuals seeking permanent residency (Green Card). 

These examinations include all elements required by the USCIS, with bloodwork, vaccines and paperwork completed onsite. 

• $495: Exam, Labs, Sealed I-693
• $445: Discount applied if using insurance to pay for labs and vaccines.
• $395: Refugee/Asylum Applicants

Please bring a valid photo ID (passport, employment authorization, license or ID).

You should be prepared with your childhood immunization records. 

If you do not have records, the doctor will scan the Maryland and DC immunization databases for your vaccine history. We provide a complimentary lab draw for immunity to MMR, varicella and hepatitis B. If you are lacking a vaccine, you can get it at our clinic with or without insurance.

If you need to pay out-of-pocket for vaccines, the following prices apply: 

$29 - Influenza        $99 - Hepatitis B

$69 - Polio                 $129 - MMR

$89 - TDAP               $229 - Varicella

In accordance with USCIS Rules, once the doctor seals your I-693 form, the document never expires!
